Are physicals required for a student entering 7th grade?
NO. A physical is NOT required when entering 7th grade unless the student is a transfer from out of state.

If District 41 moves the 6th grade to Palombi Middle School, will the new 6th grade student be required to have a current physical?
YES. Any student entering the 6th grade (no matter what school) is required to have an updated physical on file in the Health Office.

Are there any times that a student MUST be picked up from school?
YES. Any student who displays a temperature at 99.9 degrees or above, Vomiting, or Diarrhea is REQUIRED to go home.

Are students allowed to contact a parent/guardian to request to be picked up at school?
Students who are not feeling well are REQUIRED to come to the Health Office prior to contacting a parent/ guardian. If the Health Coordinator deems it necessary, the Healthcare Coordinator will contact the parent/guardian for the student to be picked up.

Are students allowed to carry prescribed Inhalers in school?
YES. Students are allowed to carry prescribed inhalers with them in school, provided proper paperwork is filed in the Health Office. “Self Carry” medication paperwork can be found in the “Forms” section of the Health Tab.

Are students allowed to carry any “over the counter” type medication in school?
NO, Students are not allowed to bring or carry any “over the counter” type medication in school. This includes pain medication, cough drops, etc. Medication prescribed by a doctor (including over the counter meds) may be kept locked in the Health Office. Proper paperwork must be included with the medication. Medication paperwork forms can be found in the “Forms” section in the Health Tab.

Are students required to provide a note from a doctor in order to be excluded from participating in PE class.?
YES. The student must present a note to the Health Office indicating the reason for the exclusion, the duration of the exclusion, and any special modifications that may be required during or after the exclusion. A parent, guardian, or the student may request a 1-day maximum exclusion without a doctor’s note.

Are students required to have a physical on file prior to participating in sports?
YES. If a student is planning on participating in either Intramural or Intermural sports, they MUST have a physical on file in the Health Office PRIOR to “try outs” for the sport. The student may have either a current “School Physical” or an IHSA/IESA Physical. (The IHSA/IESA physical is good for 395 days) as compared to the normal School Physical. Physical forms can be found in the “Forms” section in the Health Tab.
